Our History
The Health Partners Initiative was established through the combined efforts of two groups - The Public Health Foundation and Community Health Partners.
The Public Health Foundation concentrated on developing resources for unmet community health needs. The Community Health Partners focused on identifying gaps in local health services. In late 2000, the two groups decided to merge to address a broader mission of assessing needs in the community and developing the resources to address them.
What is a Healthier Communities Initiative?
Healthy Communities Initiatives have been growing nationwide, in an effort to tackle the complex problems that no single program, institution, or grant can address alone.
In addition to the goal of improving health, they go beyond a simple focus on health and medical care. They also look at jobs, education, housing, civic participation, transportation and other parts of community life that underlie health in a more comprehensive way.
Health Partners used the recommendations of the community based
Healthy People 2010 Plan to target priority health needs over the next decade for Lancaster County.
Toward this end, we have worked with existing community groups and expanded participation in a broad based dialog to identify problems, and helped to develop projects to most effectively address these needs.
Where do HPI Resources Come From?
HPI is a private, nonprofit 501(c)3 corporation that relies upon the gifts of groups and individuals in order to continue its work.

Health Partners Initiative is recruiting community members to expand our citizen involvement activities in all of our projects. Please refer to individual project descriptions to learn more about specific activities and how you can help.
